Allan, a 72 year old man who lives alone, presents with a tremor in his right hand for about 6 months.  He reports it is better when he is actively trying to do something but intermittent otherwise.  He has high cholesterol, chronic mild anxiety, GERD and irritable bowel syndrome.  He takes somac, ezetemide regularly and prn metoclopramide.  He does not smoke.  He has no allergies.

Neurology 1.2 – Jane has numb feet

Jane, a 68 year old women presents to you with both her feet feeling numb.   This has been slowly worsening over 2 years.  She reports some burning on the soles of her feet but otherwise no pain or weakness.  She is overweight and slowly putting on more weight.  She denies weight loss, recurrent infections or bruising.  Her last pap smear was 1 year ago and mammogram 1 year ago, both normal. She drinks regularly but has never smoked.   She takes panadol OTC for her feet but no other medications. She is allergic to morphines.   She has been with her husband for 46 years.  On examination her BP is 160/95 and BMI 38.  Her feet reveal present but reduced sensation on the plantar surfaces of her feet.

Jane, aged 43 presents as a new patient to your clinic.  She is in distress and reports an acute on chronic migraine, the same as previous migraines.  She gets them 1-2 times a month.  She also has nausea.  She has a letter from her old GP stating she requires “pethidine” for the migraines as no other treatments work.  She denies any other medical history or medications.  She gets side effects to neurofen and has an allergy to panadol.  She does not smoke and drinks “socially”.

RACGP KFP 2016.1.15 – Kate has changes in her menstrual pattern

Kate, age 14, presents to you with her mother.  They are concerned because her periods are occurring less often, up to 6-7 weeks between periods.  Her last period was 6 weeks ago.  Her periods started around age 12, and were irregular for 6-12 months, then regular but in the last 6 months they are reducing.  She denies any period pain. 

She is quite active at school in various sports, her academic scores are very high and she is in the school debating team.  She is eating less and has lost weight because of bullying about over the last 6-9 months. She admits to “sometimes” vomiting after meals.  She has no other past medical history and no allergies.  Her BP is 90/50, HR 52 and she looks pale.  Her BMI is 18.

RACGP KFP 2016.1.14 – Sonya is having difficulty with breastfeeding

Sonya, a 23 year old first time mother presents with her two week old baby girl, Jade.  She is worried that she has poor milk supply and that the baby might not be growing.   She reports her baby feeds very often, up to 10-12 times a day, and she does not seem to feed for long.  However she does settle between feeds well.  She has ~6 soaked nappies in 24 hours and one poo.  She denies any nipple changes or pain. 

The infants weight at birth was 3.2kg.  She lost about 300 grams in the first few days which made it clear to her that breastfeeding was not enough.  However she has put this back on now and is 3.3kg.  Her neonatal exam is normal.  She does not smoke and has no allergies.  The midwife reviewed her at home 2 days ago and reported the attachment and breastfeeding technique were good.  Sonya reports “Should I change to formula?”
